This is the after-hours call. Evening MiraLAX the night before, one capful the morning of, clears stop the listed hours before. No stimulant by design — if a family has leftover Dulcolax from a colonoscopy prep, they do not take it. No pre-cleanout. No endpoint and no rescue dose: a smaller dose may not produce diarrhea and brown stool at 8 PM is not a failure — the opposite of what our colonoscopy sheet teaches, which is why families call.

After the capsule goes in — same for every band and all three permutations

What When Detail
Clear liquids2 hafter the capsule is placed or swallowed
Light meal + MiraLAX4 h1 capful in 8 oz — keeps it moving
Not passed byday 5abdominal X-ray — “not seeing it” is never reassurance

Simethicone — staff give it; families are told nothing and bring nothing

Pathway Dose Liquid first — never the bottle dropper
Office swallow~375 mgPRIMARY 5.6 mL of 40 mg/0.6 mL by 10-mL oral syringe (or 6 mL of 62.5 mg/mL). SECONDARY 3 × 125 mg (375 mg), or 5 × 80 mg (400 mg). 30 min before ingestion.
Any endoscopic deployment333 mgPRIMARY 5 mL of 40 mg/0.6 mL by 10-mL oral syringe. SECONDARY 80 mg (1.2 mL) in a 3-mL syringe, when the child will not take the 5 mL. 30 min before anticipated start, not rooming. Notify the anaesthesia provider for EVERY dose and document it.

Before any dose: screen for bilious emesis, progressive distension or obstipation → hold and call. Stricture, prior obstruction or Crohn’s without a patency study → confirm before the capsule goes in. Delay >60 min past the anticipated start → call; a repeat dose needs a physician order. Retrieval is mandatory — the capsule stores the images, it does not transmit; kit is the silicone hat, the magnetic wand, sealed box, barcode stickers, FedEx envelope. Stock: ONE liquid concentration per site, bin labelled; 40 mg/0.6 mL, 62.5 mg/mL and 125 mg/5 mL look alike and a same-volume swap is a multi-fold error — never convert a strength by volume; anything else, stop and call.
